Be sure the saucepan used to melt the chocolate is clean and dry. Even a single drop of water in melter chocolate can cause it to seize or harden into clumps.

i always thought that this was true until I saw a chef on TV adding water to chocolate as he melted it. I have no idea why it worked but never dared to try it myself!
